Sermorelin is a synthetic peptide consisting of the first 29 amino acids of human growth hormone-releasing hormone (GHRH), which naturally contains 44 amino acids. It was the first GHRH analog approved by the FDA (under the brand name Geref) for the diagnosis of growth hormone deficiency in children. While Geref is no longer commercially manufactured as an FDA-approved product, Sermorelin remains one of the most widely studied GHRH analogs in research.

Sermorelin stimulates the pituitary gland to produce and release the body's own growth hormone, while exogenous HGH (human growth hormone) replaces it directly. This is a fundamental distinction in research. Sermorelin works upstream by activating the GHRH receptor on pituitary somatotroph cells, which triggers endogenous GH secretion in a pulsatile, physiologically natural pattern. Direct HGH administration bypasses the pituitary entirely and produces a non-pulsatile elevation.

Sermorelin research spans growth hormone physiology, aging, body composition, and sleep architecture. Published studies have investigated its effects on GH secretion patterns, lean body mass, fat distribution, bone mineral density, and sleep quality. The sleep connection is notable: GHRH activity has been shown to decrease wakefulness and increase slow-wave (deep) sleep in research models, and improved sleep is often the earliest observable effect in clinical studies.

All three are growth hormone secretagogues, but they work through different mechanisms. Sermorelin is a GHRH analog (activates the GHRH receptor directly). CJC-1295 is also a GHRH analog but has been modified for a longer half-life. Ipamorelin is a GHRP (growth hormone releasing peptide) that works through the ghrelin receptor. Sermorelin has the most extensive clinical research history but the shortest half-life, requiring more frequent administration in research protocols.
